INDEMNITY POLICY
CENTURION BALLET ACADEMY
WHEREAS Ballet, Modern, Contemporary Ballet and Jazz training can cause injuries, even serious injury ;
AND WHEREAS, by entering the premises at 1005 Pretorius Avenue, Lyttelton Manor, a dancer or parent / guardian may be injured due to the use of any of the amenities, including the swing ;
NOW THEREFORE the parent / guardian hereby indemnifies Centurion Ballet Academy and their employees or agents against all liability for any injury or loss sustained by such dancer, parent / guardian on account of any act or omission by any person and the parent / guardian accepts full liability and responsibility for any such act or omission.
The parent / guardian hereby gives permission for him/her to participate in the dance activities.
The parent / guardian hereby cedes his / her right to Centurion Ballet Academy, should medical treatment or surgery to the dancer be deemed necessary. The parent / guardian gives full permission to Centurion Ballet Academy to authorise emergency medical treatment in an emergency of his / her child and also to administer minor first aid if needed. The parent / guardian will be responsible for the payment of all medical treatments.
The parent / guardian will pay for any damage to persons or property which is caused through the misconduct or carelessness of his / her child.
The parent / guardian will not hold Centurion Ballet Academy or any employee or agent responsible for any loss of personal effects of the dancer.
The parent / guardian warrants that the dancer is physically capable of participating in the various activities aforesaid.
No transportation of any dancer is done by Centurion Ballet Academy. In cases where transport is provided, it is done by an independent contractor.
